How do I get my records transferred?
Complete and mail/ drop-off at your last doctor’s office “Transfer of Record” form found on this website or requested from this office (or hand-write your request with our address and telephone #)
Can my entire family be seen by the same doctor?
Yes, Dr. Selke is a Family Physician who takes care of the entire family, from newborns to geriatrics.
Am I always notified about my tests results, even if normal?
Yes, Our staff will always notify you of all test results. If you do not hear from us within 5 business days, please call us.
What if I have mail-order prescriptions?
Depending on the company your insurance uses, the prescription may have to be hand-written and mailed by you (please request refill 2 weeks prior to running out of the medications to allow time for mail and processing), or may be sent electronically directly to the company. Please ask the staff which method applies to your company.
Do I have to use a computer to renew my medications
or request a referral?
No. You may request both by calling the office’s main phone # (908-281-1199) and pushing the option for Refill or Referral requests, and leaving a detailed message for doctor approval. If it’s an emergency referral or you are out of your medications, please let us know and we will do our best to accommodate you.
Can I have copies of my test results or labs/ referrals/ lab requests/ diets/ patient information mailed to my home?
Yes. You may request any of these items mailed to you if you provide self-addressed stamped envelopes (we will keep these on file for you if you prefer to have items mailed regularly). The rising cost of stamps prohibit us from providing this service regularly, especially in light of the number of requests we receive daily. Otherwise, we ask that you pick-up any correspondence.
