Office Policies
Appointments:
Patients should contact the office to schedule appointments for all acute care needs, preventative care and emergencies. Appointments are available Monday through Friday, Tuesday evenings, and select Saturdays. Emergency same-day appointments are available. Please call at 9 A.M. for availability that day. If you are a new patient in this office location, we recommend you complete your registration form on this website. In addition, please read our HIPPA “Notice of Privacy Practices” and our “Financial Policy”, print and sign both, and bring this with you to your first visit to save time on arrival. However, for your convenience, the forms are available at our office as well. We require at least 24 hours notice for cancellation. If 24 hours notice is not given, or you arrive more than 30 minutes late for your appointment, you will be seen at the discretion of the doctor, and a $25 “no-show” charge may apply. If your insurance company requires a primary care doctor selection, it is your responsibility to change to Dr. Melissa Selke as your doctor before your first appointment. If Dr. Selke was your doctor at her previous office location, you must still notify your insurance of the change to her new location and phone number. If you need any assistance, our office staff is available to help in this process.
After Hours/Weekend Care:
For emergencies when the office is closed, please call the main number 908-281-1199 for the Answering Service operator. They will page the doctor on call, who is available 24 hours a day, 7 days a week by telephone. If there is a disruption of phone service, please contact the office via the operator at Somerset Medical Center (908) 685-2200.
Refills:
Refills can be requested on this website and submitted directly to our office. You can also call the office and press Refill option to leave a message with your request. Please state and spell your name, medication, dosage, and pharmacy name and phone number. Please try to call prior to running out of your medications, and allow up to 48 hours for your request to be processed. All prescriptions are sent electronically by fax to your pharmacy, except for mail-order pharmacies and regulated medications. These must be hand-written by our office and mailed to you, or faxed if the appropriate paperwork is provided.
Referrals:
Referrals can be requested on this website and submitted directly to our office. You can also call the office and press the Referral option to leave a message with your request. Please state and spell your name, the reason for the request, the specialist’s full name and address, and if known, any special requests. Please allow 5-7 days for processing, except for emergencies. Per insurance company regulations, follow-up referrals cannot be issued without a plan of treatment from the specialist, which is a letter from the specialist on our records indicating your diagnosis, follow-up plans, and need for follow-up visits. To improve communications with the specialists, please ask them at the time of your visit to send a letter to our office.
Test Results:
Patients will be notified of all test results, whether normal or abnormal, by our office staff. If you have not been contacted within 1 week of your blood being drawn, please call our office so we can contact the lab for your results. According to HIPPA confidentiality regulations, results can only be given to the patient (not spouse, significant other, etc.), unless written consent is obtained for the chart. If the patient is a minor, results can be given to the parent or guardian, with the exception of gynecological or STD-related results.
